Bacterial infections, particularly infections caused by bacteria and/or fungi, are highly prevalent and associated with high morbidity and mortality, and therefore, antimicrobial resistance is crucial in the treatment of infections. Currently, there is an urgent need for new and effective antimicrobial agents that have a higher efficacy and lower cost, thereby improving the treatment outcomes and reducing healthcare costs. To further explore the effectiveness of Doxycycline in the treatment of infections caused by bacteria and fungi, this study aims to investigate the efficacy of Doxycycline for the treatment of bacterial infections, focusing on the mechanism of action and the clinical outcome.
A cross-sectional study, conducted in a tertiary care university hospital (UH) in the year 2018, was designed with the objective to explore the effectiveness of Doxycycline in the treatment of bacterial infections caused by bacteria and fungi. A total of 4,831 patients with bacterial infections between January 2014 and June 2018 were enrolled in this study. Participants were randomly assigned to one of four groups: group A, Doxycycline treatment group (n=4,831), group B, Doxycycline therapy group (n=4,831), and group C, Doxycycline therapy group (n=4,831). Doxycycline was administered orally with a single 100 mg dose to the participants and their clinical outcomes were evaluated. The patients were divided into two groups. Group A received the oral dose of 200 mg and Doxycycline (100 mg) orally twice a day for seven days. The patients in group B received the same dose of 200 mg and Doxycycline (100 mg) orally for seven days. The patients were evaluated for clinical outcomes and bacterial eradication rate (BEE) using a modified semianalyzer (MSA) test.
The mean ± standard deviation of all variables, and the number and percentage of patients were 16.44 ± 7.08, 16.00 ± 7.18, and 16.33 ± 7.16, respectively, in group A, B and C groups. There were no significant differences between the groups on demographic data, bacterial eradication rate (BEE) at seven days, clinical outcome, and clinical signs at seven days. Compared with the group A, the group B and C groups showed significantly higher treatment success rates (80.99% vs. 76.29%) and higher clinical signs (32.44% vs. 16.99%) at seven days. In Group B, the group C group was more effective than group A and B and more effective than group C. The MSA test showed that the efficacy of Doxycycline was significantly higher in the group B than in the group C. However, the mean clinical signs at seven days, BEE at seven days, and MSA at seven days were not significantly different between the two groups. The most common side effects of Doxycycline were vomiting, diarrhea, nausea, abdominal pain, headache, insomnia, and rash. No significant difference was observed between the two groups on other outcomes. However, most of the adverse events related to the treatment success were mild and transient in the group C and group B. In addition, the patients in group C experienced mild diarrhea, nausea, nausea, abdominal pain, headache, insomnia, and rash, and the groups showed a higher incidence of severe side effects (abdominal pain, nausea, vomiting, diarrhea, and respiratory depression) at seven days compared with the group A. The group C showed a more satisfactory treatment success rate than the group A. The MSA test showed that the efficacy of Doxycycline was significantly higher in the group B than in the group A and B and more effective than group C. However, the groups showed a higher incidence of severe side effects and adverse events.
This study revealed that Doxycycline was effective in the treatment of bacterial infections caused by bacteria and fungi in patients with urinary tract infections (UTIs). The results suggest that the use of Doxycycline in the treatment of bacterial infections, particularly UTI infections, is effective in improving the treatment outcomes. The Doxycycline treatment was shown to be more effective than a single dose of 400 mg of Doxycycline in the treatment of bacterial infections.
